APPLICATION FOR OFFERING
ASHA CONTINUING EDUCATION UNITS
AT AN EVENT

Dear Conference Organizer

Thank you for your interest in offering ASHA CEU’s at your conference. In working with the Colorado Speech Language and Hearing Association (CSHA) to offer CEUs, you are effectively asking CSHA to co-sponsor your activity with you.

GENERAL DIRECTIONS

Prior to your CEU activity:

1. Complete the Activity Registration Form, print the form, and return it to CSHA, PO Box 345, Sedalia, CO 80135. Please include a check for $350 written out to CSHA to cover the cost of state and national processing fees. ($100 CSHA/$250 ASHA) Partial or incomplete registration forms will not be processed and payment must be included. Once the form is received and processed by CSHA, you will receive a packet of additional information.

2. Examine and Complete the Information Packet
The additional information requested in the packet needs to be completed and returned to the CE Administrator, Polly Somers, at the CSHA office, as requested, 60 days prior to the date of the conference.  After the application is processed by the CE Administrator it will be forwarded on to ASHA.

The packet will contain:

· General information on Continuing Ed Requirement for ASHA Certification for SLP’s

· General directions for completing application process and follow-up

· A copy of the Activity Registration Form you filled out (2 pages)

· Required promotional paragraph with blanks to be filled in as appropriate for your activity (this should be checked by CE administrator before it is printed in a brochure)

· Copies of various sizes of ASHA logo for use with paragraph in brochure or on registration table

· ASHA CEU Participant Form (1 copy): After your activity had been approved, please fill in the Provider Code (CSLH), an activity number that you will get after your application packet has been returned, the Course Title that is on the Activity Registration Form, and the completion date of the activity (date of the last session). After filing in this information, make enough copies of this form so that there is one for each participant who wants CEUs. Be sure that all 4 black squares in the corners of the form appear on the copies or the computer will not be able to read them.

· Cooperative Agreement Form (1 copy): Complete “Information About the Other Party” which is the organization offering the activity for which CEUs are being applied.

· Multiple Offerings List: To be used only for activities offered more than once during the same calendar year. CEUs can be offered presentations at no extra cost but the appropriate papers need to be filed with the initial application. If you have any questions contact Polly Somers (720) 733-9097

3. Include ASHA statement and logo in your brochure if possible.
If this is not possible, you will need to include a copy of the statement & logo on your registration table. Include a typed copy with your application,

At your activity:

1. Have participants who want CEUs sign in on your attendance sheet before the
first morning and afternoon session. Please remind participants to remember to sign in. If their names are not on the form at the beginning of each morning and afternoon, they won’t be given credit for being there and won’t receive CEUs.

Be sure that participants sign-in on the same lines every time so that the CEU administrator can find their names without unnecessary searching.

2. Have each participant complete the ASHA CEU Participant Form. Remind them to complete the whole form, including their ASHA number or their Social Security number, forms without one of these will not be forwarded to ASHA and the participant will not receive CEUs.

3. All participants should complete Learner Assessments and Course evaluations.

4. Collect ASHA CEU Participant Forms, Learner Assessments and Course Evaluations at the end of your activity.

After your CEU activity:

1. Check that required information is completed on ASHA CEU Participant Forms. Alphabetize completed forms and return them to CSHA within 14 days of the completion of the activity. Include the total number of participants in your return packet.

2. Learner Assessment and Course Evaluations are for your information and do not need to be returned to CSHA or CE Administrator.

CSHA and the ASHA CE Administrator will complete the necessary paperwork and submit the forms to ASHA. ASHA will make sure everything is complete, enter CEUs on member files, and send a completed list of the participants who were awarded CEUs to the CE Administrator. The CE Administrator will forward that list to you. Neither ASHA nor CSHA is responsible for notifying participants. Participants can check with ASHA after 3 months to make sure CEUs were awarded.
