Details of the CSH Foundation

 Created
in 1984, the CSH Foundation is a philanthropic, 501(c)3 public nonprofit
organization. The foundation assists the speech-language-hearing
profession and its consumers by providing educational opportunities for speech-language
pathologists and audiologists, supporting research, and direct
assistance to people with communication disorders.
Current
board members, who are responsible for all decision-making regarding the
foundation's activities and fund-raising, are listed below.
Goals
of the Foundation
The
goal of the Colorado Speech-Language Hearing (CSH) Foundation is to promote an
improved quality of life for children and adults with communication
disorders.

Activities
Recent
achievements of the Foundation have included:
- Six
graduate level scholarships to assist in education of Colorado students in
speech-pathology and audiology
- One
professional grant awarded to a working professional to begin a summer
camp for school age children with dysfluent speech
- Ten
honorariums for speakers at the annual speech-language and hearing
convention for families and professional enrichment
- Co-sponsorship
with CSHA of numerous workshops, including the May 1999 Spring meeting on
"Use of the Internet in Speech Pathology and Audiology," including
communication, research, and creation of materials.
The
Foundation's fund raising activities have included:
- Educational
opportunities through workshops provided to professionals in areas such as
voice disorders, oral-motor speech problems, and educational audiology.
The workshop speakers have given generously of their time and talent.
- Raffles,
social activities, and networking activities.
- Private
donations from individuals used as memorial contributions, professional
recognition awards, and direct contributions.

Foundation is a tax exempt organization under 501(c)3 of the Internal

COLORADO SPEECH-LANGUAGE HEARING FOUNDATION
PROFESSIONAL STUDIES GRANT
2008-2009
PURPOSE
The Colorado Speech-Language-Hearing
Foundation invites regular members of the Colorado Speech
Hearing Association (CSHA) to apply for a professional studies
grant of up to $800.00 to be awarded in June 2008 to further
their professional development. The intention of this grant is
to advance the skills of certified professionals through a
course of specialized study or certified training. The grant may
be applied to a course held between June 2008 - May 2009.
The selection will be based on a
description of the course of study and how it will further their
ability to deliver services to their clients, students or
patients. The course of study must be completed between June
2008 - May 2009.
It is expected that the individual
selected will share the information with other members of the
association. This may include a presentation at a CSHA
meeting/convention, seminar or workshop.
SUBMISSION PROCESS
All proposals for the grant must be sent
to the Foundation. All proposals must be received by May 15,
2008. The recipient will be notified by June 15, 2008.

COLORADO SPEECH-LANGUAGE HEARING FOUNDATION
ELIZABETH G. JANCOSEK GRADUATE STUDENT SCHOLARSHIP
The Colorado-Speech-Hearing Foundation scholarship is a
$500.00 nonrenewable scholarship offered to eligible graduate students.
ELIGIBILITY: Applicants must be enrolled as a
full-time student and must be candidates for a Masters degree or Ph.D. in
the fields of speech/language pathology, audiology and/or speech and hearing
science at a graduate school in Colorado. Applicants must be residents in
the state of Colorado and show evidence of outstanding ability and
application of learning in clinical and/or research endeavors.
APPLICATION: The completed application form must be
accompanied by the following supporting documents.
- Unofficial graduate transcript
- Letter of recommendation from advisor or department chairperson documenting
the applicant's graduate status
- Evidence of Colorado residency ( Colorado drivers
license, voter registration)
- Three reference letters
- A brief essay, not to exceed 300 words, written by
the applicant which indicates their interest in and plans for a career
in speech-language pathology or audiology.
- Résumé
- Enclose three copies of the essay and scholarship
application form.

of each year. Notification of the award will be made by

CSHF reserves the right not to award a scholarship if no
