  • Maximizing the Motor Speech Evaluation: A Crash Course for the Acute Care Speech-Language Pathologist - 90-Minute

    Abstract:
    Dysarthria is a disorder of the execution of speech which can negatively affect speech intelligibility naturalness with detrimental effects on communicative participation and quality of life. Dysarthria etiology includes numerous neurologic diseases, such as stroke, traumatic brain injury, Parkinson’s disease, multiple sclerosis, and amyotrophic lateral sclerosis. Despite this relationship, motor speech diagnostics are often under-utilized in the busy acute care setting. The potential contribution to differential neurologic diagnosis may therefore be overlooked. In addition, dysarthria patients report under-referral to speech therapy services, suggesting an assessment gap may negatively impact future intervention. This presentation will overview evidence-based procedures for motor speech assessment, propose a framework for efficient interpretation of evaluation findings and suggest directions for treatment planning.

    At the completion of this webinar, participants will be able to:

    • Identify and list neurological conditions associated with subtypes of dysarthria.
    • Describe symptoms associated with dysarthria subtypes and associated neurological conditions.
    • Create relevant goals based on a subsystem approach.

    About the Presenter:
    Allison HilgerAllison Hilger, PhD, CCC-SLP is the Principal Investigator of the CO Motor Speech Lab at the University of Colorado Boulder. She started at CU in 2020 after graduating with her PhD from Northwestern University. Allison’s research interests include auditory feedback control in speech, phrasal prosody in dysarthria, and the acoustics of speech for aerosol generation. In her free time, Allison enjoys hiking with her dog, Bader, her husband, Daniel and her son, Benjamin in the many trails around Boulder, Colorado.  She also enjoys reading, skiing, and weightlifting.

    Abstract:
    Voice professionals may encounter patients and clients experiencing emotional distress. Trauma is a psychosocial risk factor in patients with functional voice disorders (FVDs), which are functional neurological disorders (FNDs) with “speech symptoms” (American Psychiatric Association, 2022). In FNDs, patients’ primary complaints are usually somatic symptoms though they may have co-occurring psychological stressors and dissociative symptoms. Patients and clients may not feel comfortable disclosing a trauma history. Practitioners, however, can practice trauma-informed voice care with or without trauma inquiry or disclosure.

    Learner Outcomes:
    At the completion of this webinar, participants will be able to:

    • Identify and explain how social determinants of health (SDOHs) may contribute to trauma symptoms in patients with functional neurological disorders with communication symptoms such as muscle tension dysphonia.
    • List the Substance Abuse and Mental Health Services Administration’s (SAMHSA) four assumptions of trauma-informed care.
    • Recognize how trauma-informed practices may advance health equity for vulnerable populations seeking voice care.
    • Recognize relevant ethical considerations within the SLP scope of practice and competence.
